Ovulation Pain Every Month — Why It Happens and What the Pattern Is Telling You

If you get a sharp pain on one side every month like clockwork, your body is giving you precise ovulation data. Most women who experience it don't realise that's exactly what it is.

What's actually causing the pain

The pain is called mittelschmerz — German for “middle pain,” named for when it appears in the cycle. It occurs when a follicle on one of your ovaries ruptures to release an egg. That rupture releases a small amount of fluid and blood into the pelvic cavity, which can irritate the surrounding tissue.

The sensation varies between women and between cycles. It can be a brief, sharp twinge that lasts seconds, a dull ache for a few hours, or occasionally an uncomfortable cramping that lasts into the next day. All of those are within the normal range for mittelschmerz. The key is that it's tied specifically to the moment of follicle rupture.

Why it happens every cycle

Ovulation happens every cycle in which you ovulate — which, for most women of reproductive age, is most cycles. So if you feel it every month, you're not experiencing anything abnormal. You're simply someone who is sensitive enough to the physical process of ovulation to feel it.

Pain intensity varies because follicle size, fluid volume, and individual pelvic sensitivity all differ cycle to cycle. A larger follicle may produce more sensation on rupture. Cycles with higher oestrogen can sometimes produce stronger mittelschmerz. This is why some months feel stronger than others even when the timing is consistent.

Some women always feel it; many never do. Both are completely normal. The presence or absence of mittelschmerz tells you nothing about your fertility or your ovulation quality — only about your sensitivity to the process.

Why it might always be on the same side

Ovaries don't alternate strictly on a schedule. Over time, most women will feel pain on both sides across different cycles — but it's common to have a dominant side, particularly if one ovary tends to produce the lead follicle more often. This isn't a problem in itself.

Persistent one-sided pain — particularly if it's intensifying over time or present outside of ovulation — is worth paying attention to. Endometriosis, ovarian cysts, or adhesions can all cause chronic unilateral pelvic pain that might be confused with mittelschmerz. The distinction: mittelschmerz is brief and tied to mid-cycle. Pain that lingers, worsens progressively, or appears at other points in your cycle is a different signal.

Women with PCOS sometimes experience more pronounced ovulation pain when ovulation does occur — particularly if the follicle that finally releases has been developing for longer than usual due to delayed ovulation. If your mittelschmerz is intense and your cycles are irregular, PCOS is worth investigating.

When the pattern matters

Consistent timing is your most useful data point. If your pain appears reliably on cycle day 13–15 every month, that's a reliable ovulation signal — and it tells you something genuinely useful about your fertile window that a calendar prediction can't.

Changes in the pattern matter more than the pain itself. If it's been predictably arriving on day 14 and suddenly starts arriving on day 20, that's information about your cycle changing. If it's been mild for years and suddenly becomes intense, that's also worth noting.

Paired with other ovulation signs — cervical mucus changes, a basal body temperature shift, energy and mood shifts — the mittelschmerz becomes one piece of a larger picture that's far more useful than any of these signals alone.

What to track alongside the pain

Which side

Left or right. Over several cycles, this tells you whether your ovaries are alternating or whether one is consistently dominant.

Cycle day it appears

Day 1 is the first day of full flow. Knowing which cycle day your pain appears tells you a lot about when you're actually ovulating.

Duration

A few hours is typical. Pain lasting more than 24–48 hours is worth noting and discussing with a doctor if it becomes a pattern.

Intensity

A simple 1–5 scale works. Changes in intensity across cycles can indicate shifts in follicle development, oestrogen levels, or other cycle factors.

Other symptoms the same day

Discharge changes, mild bloating, energy shifts, or mood changes on the same day as mittelschmerz help confirm ovulation and build a fuller picture.

When to see a doctor

Mild mittelschmerz that resolves within 24 hours and appears consistently mid-cycle is normal and doesn't require medical attention. Most women who experience it manage it with rest, a heating pad, or over-the-counter pain relief if needed.

See a doctor if the pain lasts more than 2–3 days, is getting progressively worse across cycles, is accompanied by fever, nausea, or vomiting, or if you have unusual or heavy discharge alongside the pain. These can indicate conditions — including ovarian cysts, ectopic pregnancy, or infection — that need evaluation.

Also see a doctor if your pain is severe enough to significantly disrupt your daily life. Mittelschmerz that requires strong pain relief or prevents normal functioning isn't something to simply endure. Endometriosis in particular can cause ovulation-phase pain that looks like mittelschmerz but has a different underlying cause and treatment.
